This form is used to assess suitability for supported housing or a floating support service in Brent, or a referral for Move On to independent accommodation.
Information provided will be used to match the support needs of the service user to the most suitable service available at the time of the referral, and for monitoring purposes to improve our services.
Where an Agency is completing the form on behalf of a client, the client’s signature is required, to indicate their consent has been given for information to be shared, and for the application to be processed.
Please be aware that there are not enough council homes available for everyone moving on, and for most people the only available option will be private renting. Your Support Worker will discuss the options with you. Please see the Help panel for details of how to complete and where to send the form.
